Job Summary of Unclassified:

This role involves providing in - home clinical assessments to patients, focusing on preventive care and improving overall well - being. By conducting comprehensive evaluations in the comfort of patients homes, you will help identify health risks, coordinate follow - up care, and support individuals in navigating the healthcare system. Flexible scheduling, excellent compensation, and no nights, weekends, or on - call shifts make this position a balanced and rewarding opportunity.

Job Duties and Responsibilities of Unclassified:

• Conduct in - home assessments including medical history review, medication reconciliation, vital signs, and physical exams.
• Perform screenings and point - of - care testing as appropriate.
• Identify diagnoses for care management and treatment planning.
• Document current and past medical conditions using clinical expertise and findings.
• Communicate assessment results to primary care providers to close gaps in care.
• Recognize urgent or emergent situations and provide appropriate intervention.
• Educate patients on disease processes, medication usage, and health compliance.

Qualifications and Experience of Unclassified:

• Active and unrestricted Nurse Practitioner license in the state of Georgia.
• Graduate of an accredited NP program with national certification.
• Strong clinical assessment and communication skills.
• Ability to work independently in patient homes across assigned territories.
• Valid driver’s license and reliable transportation.
• Commitment to patient - centered care and preventive health practices.
